Soben Management Ltd. collects personal and medical information about you and your dependants on behalf of the UNITE HERE Health & Welfare Plan, the UNITE HERE Wellness and Dental Centre, and their service providers. The information collected is needed to properly run the Plan and to comply with applicable laws. These tasks may include:
- Enrolling you in the Plan,
- Determining your eligibility and benefit coverage,
- Assessing and paying claims you submit,
- Providing you with a drug card,
- Allowing you to visit the UNITE HERE Wellness and Dental Centre,
- Communicating with you about your benefits,
- Protecting you and the Plan from fraud and error,
- Verifying and auditing claims, eligibility and contributions,
- Underwriting activities, such as determining the cost of current and proposed benefits,
- Preparing tax slips and regulatory reports.
Soben may exchange personal or medical information about you or your dependants with respect to your benefits with your health care providers, and with insurance companies and drug card providers working for the Plan. Personal or medical information about you will only be disclosed to outside parties with your permission, or as required by law.
